Ibogaine + 5-MeO-DMT: Understanding the Combination Protocol

The ibogaine and 5-MeO-DMT combination is one of the most discussed — and most misunderstood — protocols in the psychedelic treatment space right now. Some clinics offer it as a standard retreat format. Others are skeptical, citing the lack of controlled research and the compounded physiological demands.

The honest answer: the combination protocol is real, used by a growing number of clinics in Mexico and other legal jurisdictions, and there are thoughtful reasons why some practitioners use it. But the evidence base is still primarily observational, the physiological risks require careful management, and quality varies dramatically depending on who’s running it.

What the Combination Protocol Is

Ibogaine is administered first. The ibogaine experience typically lasts 24–36 hours — an extended introspective state with intense biographical review. For opioid-dependent patients, ibogaine simultaneously interrupts acute withdrawal through its action on kappa-opioid receptors, NMDA receptors, sigma receptors, and the serotonergic and dopaminergic systems.

After a 24–72 hour recovery period, 5-MeO-DMT is administered. The 5-MeO-DMT experience lasts 15–45 minutes and is described as a dissolution of ego boundaries — a non-visual but intensely felt state of unity. The two experiences are profoundly different: ibogaine is the long, difficult excavation; 5-MeO-DMT is the brief, oceanic opening that follows.

Why Clinics Use Both

The theoretical rationale comes from the distinct but potentially complementary mechanisms of each medicine. Ibogaine surfaces and processes deep psychological material. Many patients finish the ibogaine journey feeling they’ve worked through something but are exhausted and raw — not yet integrated.

5-MeO-DMT, administered after the patient has rested, produces what many patients describe as a sense of release or dissolution — a feeling of being held by something larger than the personal story ibogaine surfaced. Practitioners describe it as ibogaine doing the excavation, 5-MeO-DMT providing the cleanse.

The research connection: studies of psilocybin-assisted therapy by Matthew Johnson, Roland Griffiths, and colleagues at Johns Hopkins found that the mystical-type experience — unity, transcendence, deeply felt significance — is one of the strongest predictors of lasting therapeutic benefit. Many practitioners believe 5-MeO-DMT is particularly efficient at producing this type of experience.

What the Experience Is Like

Day 1 — Ibogaine: Begins 1–2 hours after dosing. Most patients enter a long, visionary, often challenging state lasting 8–24 hours at peak intensity with another 12–24 hours of residual effects. Sleep is usually impossible during the acute phase.

Days 2–3 — Recovery: Patients rest and begin processing. Medical monitoring continues as ibogaine’s effects on cardiac conduction can persist.

Days 3–4 — 5-MeO-DMT: After adequate rest and medical clearance, 5-MeO-DMT is administered by inhalation. Onset is immediate. Complete dissolution of self-concept within 30–60 seconds. Peaks within 5–10 minutes, resolves within 30–45 minutes. Integration conversations follow.

Evidence Base

Honest assessment: no randomized controlled trial has evaluated the combination protocol specifically. What we have is case series from clinics like Crossroads in Mexico (Dr. Martin Polanco has treated hundreds with this protocol), practitioner-level observations from Dr. Octavio Rettig and colleagues on 5-MeO-DMT in therapeutic settings, and the underlying evidence for each compound individually — including the landmark 2024 Stanford Nature Medicine study on ibogaine for veterans.

Anyone claiming the combination is definitively superior to ibogaine alone based on published evidence is overstating what we know. The rationale is coherent and clinical experience is consistent enough to take seriously — but the science is still catching up.

Who Is This Appropriate For?

The combination tends to be most relevant for: patients with addiction plus co-occurring trauma or PTSD; veterans with PTSD, TBI, and substance use disorder; patients who have done ibogaine previously but want deeper integration; individuals seeking treatment for depression or existential distress.

Less indicated for: patients who have never had psychedelic experience, are highly anxious, or are primarily seeking opioid detox without significant trauma complexity. In those cases, ibogaine alone with strong integration support may be the more appropriate starting point.

Contraindications and Extra Cautions

Cardiac screening is mandatory. Ibogaine prolongs the QT interval. Any cardiac condition, particularly arrhythmia or QT prolongation, requires proper screening. This cannot be skipped. See our Ibogaine Safety Guide for full detail.

Serotonin risk. 5-MeO-DMT is a potent serotonin agonist. Patients on SSRIs, SNRIs, MAOIs, or other serotonergic medications face serious serotonin syndrome risk. Full medication washout before either compound is non-negotiable.

Timing must be respected. Responsible clinics enforce a minimum 24-hour rest period between medicines, typically 48–72 hours. Less than 24 hours is a red flag.

How to Evaluate a Clinic Offering This Protocol

Ask: Who is the medical director and what are their specific ibogaine credentials? What does cardiac screening involve? What is the exact timing between ibogaine and 5-MeO-DMT? Who administers the 5-MeO-DMT and what is their training lineage? What does integration support look like before and after the retreat?

Psychedelic Connect evaluates clinics against these criteria before making any referral. We know which programs use the combination protocol, who their medical staff are, and what their track record looks like.

Cost

Combination retreats at established Mexican clinics typically run $5,000–$15,000 for the full program. The $5,000 floor is a negotiated rate that applies only where no detox is required and you are not currently using; most people should plan against the middle of the range. Higher-end programs with more intensive medical oversight and multi-week integration can run $15,000–$25,000. Not covered by U.S. insurance.

Our patient advisory service

At Psychedelic Connect, we help patients find clinics. We’ve spoken with medical directors, reviewed protocols, and compiled what we know about which programs offer the combination and what their standards look like.

Call us at (866) 435-7057 or visit our consultation page. There is no obligation, and we don’t refer patients to clinics we wouldn’t refer our own family to.

Frequently Asked Questions

Is ibogaine and 5-MeO-DMT safe in combination?
A: When properly administered with appropriate timing, cardiac screening, and medication washout, yes. Clinics that skip screening or rush the timeline are the primary safety concern, not the combination itself when done correctly.

How long does the full combination retreat take?
A: Most programs run 5–8 days. Some programs with intensive integration extend to 10–14 days.

What if I only want ibogaine without 5-MeO-DMT?
A: Many excellent clinics offer ibogaine as a standalone protocol. The combination is not necessary for effective ibogaine treatment. We can help you find either type of program.

Is toad-derived 5-MeO-DMT the same as synthetic?
A: The active molecule is the same. Toad venom contains additional alkaloids. Many programs now use synthetic 5-MeO-DMT for consistency and to avoid animal welfare concerns.

How do I know if the combination is right for me?
A: That’s a conversation to have with experienced practitioners who know your history. Call (866) 435-7057 — we’ll help you think through the variables and help you find programs that fit your specific situation.
